All score forms must be signed by both coaches, the official and the supervising administrator of the home team.

In the event of an Ejection, coaches are to complete the EJECTION INCIDENT REPORT FORM, available on the website and submit to CSHSAA Executive Director Leah Conforti (leah.conforti@cssd.ab.ca) and no later than 9:00 am next day.
