CONSTITUTIONAL APPLICATION FORMS

• Applications must be submitted to the CSHSAA Executive Director by the Principal of a member school only (except the Perceived Member Violations Application Form, which can be submitted by any individual). Emails to be sent to Executive Director Leah Conforti at leah.conforti@cssd.ab.ca

• For those Applications that must be submitted by the Principal, if the Application is not emailed directly by the Principal, it must contain the Principals' scanned signature; If the Application is emailed directly by the Principal, this will be accepted as the electronic signature.

• If the Appeal Form is not emailed directly by the Principal, it must contain the Principals' scanned signature; If the Appeal Form is emailed directly by the Principal, this will be accepted as the electronic signature.
